Recruitment & Membership
COR Membership Acceptance Form - please return completed form to 380 Shriver Center within 48 hours of accepting a bid
Depledging Form - please return completed form to 380 Shriver Center within 48 hours of declining a bid.
Standards
Greivance Form - Contact Amy Bush, VP of Standards for Panhellenic, regarding proper standards procedures.
Transportation Contract - Each bus driver must complete this form when driving chapters to events.
Philanthropy
Event Registration Form - Chapters must register all philanthropy and community service events prior to the event for approval. Contact Kendall McCune with further questions.
Event Follow Up Form - Please submit to Kendall McCune, 380 Shriver, 2 weeks after a registered philanthropy event.
Individual Excellence Award - Awarded to an individual who excels in philanthropic events.
Chapter Excellence Award - Awarded to a chapter that excels in philanthropic events.
