Before you attend an Outdoor Adventure Club Event, you must fill out both a "Club Sports Assumption of Risk and Release of Liability Form" and a "Club Participant Emergency Information Form"
Please turn the forms into an officer at a club meeting or into the MUOAC mailbox in the upstairs office of the Shriver Center.
